Most domestic mailings with a tracking or extra services barcode are eligible for Package Intercept. Retail Senders: You can redirect a shipment back to your address or to a Post Office™ location as Hold For Pickup. You can only request a Package Intercept online with a USPS.com account. Planned Giving - Create Your Legacy

Planned Giving – Create Your Legacy Create Your Legacy and Positively Impact Seniors for Generations To Come! Estate Planning Should Be Rewarding and Impactful Through the years, legacy gifts from caring, passionate supporters have played a vital role in the Council On Aging's (COA) success. Many of our donors choose to continue supporting COA through their estate planning. By simply remembering COA in your will and/or beneficiary designations on your IRA, 401K, Life Insurance, or other similar accounts, you can make a transformative gift that will ensure seniors in our community are able to live their best possible lives with dignity and independence for generations to come. Mary Peck's wish was to renovate her bay front home, so older adults could enjoy the bay view that she had loved during her lifetime. Unfortunately, her home was not suitable to serve that purpose. Knowing we were planning River House, Catholic Charities, who was responsible for carrying out Mary's wish, gave the proceeds of her estate to COA. That legacy gift laid the cornerstone to construct River House, a state-of-the-art senior center built on the bay overlooking the Matanzas River, St. Augustine Lighthouse and Bridge of Lions. We have received other planned gifts that have helped increase the level & number of quality programs and services offered by COA, and there is so much more to do. Please join us and our generous donors. Make a difference in the lives of seniors for generations to come! Leaving COA in your will or beneficiary designation will not impact you financially while you are alive, and can be altered if necessary in the future. Remembering COA with a Gift in your Will A gift to St. Johns County Council on Aging, Inc. (COA) in your will or revocable trust proclaims your confidence that we will continue to serve our mission and make a difference in the lives of future generations. Remembering COA with a gift in your will: • Is easy to arrange • Will not alter your current lifestyle in any way • Can be easily modified to address your changing needs. Residual Bequest Language A residual bequest comes to us after your estate expenses and specific bequests are paid. Here is sample residual bequest language: I give and devise to St. Johns County Council on Aging, Inc. (EIN: 59-1525829), a nonprofit organization with headquarters in St. Augustine, FL, all (or state a percentage) of the rest, residue and remainder of my estate, both real and personal, to be used for its general support (or for the support of a specific affiliate, fund or program). Specific Bequest Language Naming COA as a beneficiary of a specific amount from your estate is easy: I give and devise to St. Johns County Council on Aging, Inc. (EIN: 59-1525829), a nonprofit organization with headquarters in St. Augustine, FL, the sum of $__________ to be used for its general support (or for the support of a specific affiliate, fund or program). Contingent Bequest Language COA can be named as a contingent beneficiary in your will or personal trust if one or more of your specific bequests cannot be fulfilled: If (insert name) is not living at the time of my demise, I give and devise to St. Johns County Council on Aging, Inc. (EIN: 59-1525829), a nonprofit organization with headquarters in St. Augustine, FL, the sum of $_______ (or all or a percentage of the residue of my estate) to be used for its general support (or for the support of a specific affiliate, fund or program). Retirement Plan & Life Insurance Beneficiary Language You may name COA as a beneficiary of your IRA or other qualified retirement benefits. Donors should consult with their tax advisor regarding the tax benefits of such gifts. Naming COA as the beneficiary of a life insurance policy or qualified retirement plan asset such as a 401(k), 403(b), IRA, Keogh or profit sharing pension plan will accomplish a charitable goal while realizing significant tax savings. It can be costly to pass such assets on to heirs because of heavy tax consequences. By naming COA as a beneficiary of a life insurance policy or retirement plan, the donor maintains complete control over the asset while living, but at the donor's death the plan passes to support COA free of both estate and income taxes. Making a charitable gift from your retirement plan is easy and should not cost you any attorney fees. Simply request a change of beneficiary form from your plan administrator. When you are done, please return the form to your plan administrator and notify Amy Robinson, Development Manager at COA. Please use the following if you wish to make COA a beneficiary: • Legal name: St. Johns County Council on Aging, Inc. • Address: 180 Marine Street, St. Augustine, FL 32084 • Tax ID Number: 59-1525829 Remember to keep your beneficiary designation forms up-to-date, as well as your will or trust, to ensure they are consistent and reflect your current wishes. Customized Language If you or your attorney would like COA to provide you with customized beneficiary language that is specific to your goal and interest, please contact Amy Robinson, Development Manager, at COA at [email protected] or (904) 209-3700. Please call or email, so that we can assist you through every step of the process. Other Ways to Give Through Your Estate There are several other types of gifts you can give COA that will benefit both COA and you and/or your beneficiary including charitable gift annuities, charitable remainder trusts, charitable lead trusts and many more. Your financial advisor or elder law attorney can help guide you toward the best solution for you. If you believe strongly in the work being done by the Council on Aging (COA), and want to be part of the future of our success, please consider COA when you are doing your estate planning. And, thank you for your kindness and for caring about the seniors in our community who will benefit from your generosity! Please contact COA Development Manager Amy Robinson to discuss how you can create your legacy through planned giving at (904) 209-3700 or [email protected].

Donate – Council on Aging (COA)

Donate Today and Make a Difference in the Lives of St. Johns County's Older Adults Your gift helps enable older St. Johns County residents to maintain their health, happiness, and independence through the many programs and services Council on Aging offers. There are many ways for you to give back to those in our community who have already given so much throughout their lives. With your help, we can be there for them today. Consider donating securely via GiveButter to the left or donations may also be mailed to: Council on Aging at 180 Marine Street, St. Augustine, FL 32084. Leave a Lasting Legacy! Mary Peck is a Legacy Donor who is making a difference to seniors in our community for generations to come through her planned gift. You can make a lasting difference as well! Have questions? Please contact COA Development Manager Amy Robinson Call - (904) 209-3637 [email protected] Giving programs with Council on Aging: - Annual or Monthly Pledges/Donations - Legacy Gifts/Planned Giving - Friends of COA - Business Supporters - Memory Enhancement Scholarships (IMEP) - Naming Opportunities - Program Sponsorships (Meals on Wheels, Sunshine Center, etc.) - Make a QCD From Your RMD Making a QCD From Your RMD At age 70½ or older, one of the best ways to make a charitable contribution can be through your traditional IRA. You can use your IRA income rather than cash to make charitable gifts by making a Qualified Charitable distribution (QCD) to your favorite charity. All you need to do is: - Contact your IRA administrator and authorize a direct transfer from your IRA to St. Johns County Council on Aging. - Have the IRA administrator make the check payable to St. Johns County Council on Aging. - Address: 180 Marine Street, St. Augustine, FL 32084 - Phone: 904-209-3700 - If you need it, our tax ID# 59-1525829 This QCD can satisfy your required minimum distribution (RMD) for the year. It reduces your taxable income, which can help reduce your income taxes. The maximum amount for charitable distributions from your IRA cannot exceed $100,000 per calendar year. (The starting age to begin making Required Minimum Distributions (RMDs) now is 72, but QCDs still can begin at 70½.) You can also name a 501(c)(3) public charity, like the Council on Aging, as your IRA beneficiary, to enable you to fund your charitable and legacy objectives in an efficient and effective way while potentially experiencing tax savings for your heirs and your estate. Always consult with your financial advisor. All gifts are processed by St. Johns County Council on Aging, a nonprofit 501(c)(3) organization (EIN: 59-1525829)

IMEP™ for Senior Care Organizations

IMEP™ Integrative Memory Enhancement Program is an original, research-based classroom program designed to optimize cognitive functioning and slow the progression of memory loss and dementia. The program serves an urgent need for adults facing these challenges. IMEP™ is a turnkey programming solution available for use by agencies and facilities that serve adults. It is being implemented successfully throughout the United States by agencies and senior care providers who lease the program for use. The comprehensive IMEP program supports brain function through integrating mind, body, and brain connections, and incorporates: - Research-based exercises to strengthen cognitive reserve - Memory-boosting techniques - Mental agility exercises - Guided relaxation - Stress-free classroom environments - Camaraderie, laughter, and mutual encouragement Benefits to Agencies: IMEP is a complete, turnkey programming resource that provides all of the tools, materials, and training necessary to successfully implement the program at any classroom location. Programming subscription agreements provide 52-weeks of research-based, original content, phone support, and optional facilitator training at our location or yours. What Doctors & Agencies Say About IMEP™: Dr. Melanie Chandler, Ph.D., Mayo Clinic, Division of Psychology: "The IMEP program is a valuable resource for our MCI (mild cognitive impairment) and dementia community. Their approach to engaging individuals in physical and cognitive exercises in a social environment aligns well with what we know helps older individuals with memory loss. I hear many positive things about IMEP from my patients, not only that they find it helpful, but also that they find it to be 'so much fun' (as a recent patient of mine stated). I will continue to refer my patients to the IMEP program." Julie Tombari, Administrator, New Day Adult Day Center, North Palm Beach, FL: "We have been with the IMEP program for years and we absolutely love it. It brings people in who do not normally come to a day program. It has attracted a new clientele that would not come otherwise. The clients just love it. The feedback we receive from families is equally wonderful. They always report that their loved ones talk more, and they are happier and more excited by life since coming to the program. 50% of our current clientele would not be here if it wasn't for IMEP." Crystal Collier, Kane Center, Council on Aging of Martin County, FL: "We have had tremendous growth with the IMEP program. We are attracting people at the early stages of cognitive decline who are motivated to be proactive and take action to mitigate the progression. The materials provided keep the participants engaged and stimulated. It has had a huge impact on our agency and we love it." What participants and caregivers say about IMEP™: "It's a totally relaxed and non-threatening environment. It's very comfortable – one of the most comfortable I've experienced. That's just as important as the educational aspect – the experience of being in a supportive and caring environment." "From the moment we walked in, we were welcomed by the facilitators and the participants! The class agenda was introduced to Mom in such a way that would make her feel comfortable and in control of making a positive and educated choice and change in her life." "The classes have greatly improved my husband's overall ability to function. When he first arrived he wouldn't engage with other participants. Now he has a positive outlook, joins in group discussions, raises his hand to share, and interacts socially with others in the class." "I am so happy that my mom has the opportunity to be part of this exceptional program. Mom is excited, positive, and for the first time in ten years she wanted to go buy herself a new outfit to go to class!" "Mom is absolutely loving it and never wants me to pick her up early! This program, your staff, and even the precious individuals in the class, are a complete answer to our prayers! My family is so thankful for your program!" "IMEP is the one true place where my wife gets to socialize. There's no other place, not even church, where she is able to truly interact. She has real friends there. It's the days that she can go to IMEP that she comes alive." IMEP™ from Home - Online Program: COA's ground-breaking Integrative Memory Enhancement Program, IMEP™, may also be offered online. Now participants can take action to enhance memory and stay sharp through participating in IMEP™ online from the comfort of home. Interested in Learning More About IMEP™ for Your Agency? The IMEP™ program is being successfully implemented at locations that serve seniors throughout the U.S. If you are an agency interested in implementing IMEP™ for memory services at your organization, please contact COA's Brian Robinson at (904) 209-3644 or [email protected].

Independent Living

Independent Living – Council on Aging (COA) INDEPENDENT LIVING SERVICES Research conclusively shows that most seniors prefer to age in place in the comfort and familiarity of their own homes. COA can help! COA Independent Living Services offer access to home and community-based programs and services provided through contracts with a variety of federal, state, county, and local entities. These subsidized programs facilitate at-home, independent living for older adults (60+ for most programs) and are designed to offer cost savings for those who need support. Services may include Meals on Wheels, personal care, help with household tasks and errands, respite for caregivers, emergency alert systems, and mental health counseling. The services are contingent upon availability and eligibility. We are happy to tell you more about these subsidized services and how to access them. To learn more, please call our Independent Living Services department at (904) 209-3700. You may also contact ElderSource, the Northeast Florida Area Agency on Aging, to request a waitlist screening for these programs at https://myeldersource.org or by calling (904) 391-6699. For more information about programs offered, visit the Florida Department of Elder Affairs website at https://elderaffairs.org/programs-services. Have questions? If so, please call Council on Aging to learn how you may be able to receive free or reduced-cost medication offered by the pharmaceutical companies. If you are eligible, we will help you navigate the application process. The Prescription Assistance Program has been helping St. Johns County seniors since 2003. In recent years, the program has saved seniors over $3,000,000 per year! Contact COA's Prescription Assistance Coordinator Tina Brophil, RN, to learn more at (904) 797-8700 or [email protected]. Ponte Vedra Memory Center

Ponte Vedra Memory Care Center – Council on Aging (COA) 1048 A1A N., Ponte Vedra Beach, FL 32082 Susan Miller (904) 729-9535 [email protected] Mondays 9am - 12noon; Tuesdays, Wednesdays, & Thursdays 9am - 2pm The Ponte Vedra Memory Care Center is a valuable resource for individuals to proactively address memory loss or dementia through participation in COA's Integrative Memory Enhancement Program, IMEP™. IMEP™ is a dynamic and innovative response to memory loss. A structured, research-based program, IMEP integrates the mind, body, and brain connections to boost cognitive reserve and minimize the progression of memory loss or dementia. IMEP employs specific cognitive and physical exercises designed to support cognitive function. The program also utilizes healing modalities that support overall brain and body function. A light-hearted atmosphere, authentic dialogue, and engaging social interaction are the hallmarks of each and every IMEP class. Each day's class focuses on a different brain topic, for example flexible thinking, spatial orientation, communication, or reasoning. The day's activities then support and stimulate that brain function. The program provides a dignified, supportive, and non-judgmental environment in a relaxed classroom setting that encourages camaraderie. At the Ponte Vedra Memory Care Center, IMEP™ is offered in 3-hour and 5-hour program formats. The 5-hour program includes lunch. What our participants and caregivers say about IMEP™ "It's a totally relaxed and non-threatening environment. Mom is excited, positive, and for the first time in ten years she wanted to go buy herself a new outfit to go to class!" The Ponte Vedra Memory Care Center is located at 1048 A1A North in Ponte Vedra Beach, Florida. For more information and to register, please contact Susan Miller at (904) 729-9535.
