Care Connection – Council on Aging (COA)

Care Connection – Council on Aging (COA) COA's Care Connection program helps independent, older adults meet and manage the more challenging demands of everyday life. As a 100% donor-funded and volunteer-powered program, Care Connection is able to fill gaps by providing crucial services for which there are no government funding sources. These services help seniors remain in their own homes where they are most comfortable, rather than being compelled to enter assisted living for lack of assistance. Services include minor home repairs, basic lawn care, access to assistive devices, installation of safety and assistive equipment such as grab bars and wheelchair ramps, food for pets, help with monthly budgeting, and help applying for state and federal assistance programs. Care Connection's office is located at COA's headquarters at 180 Marine Street in downtown St. Augustine, Florida. Our phone number is (904) 209-3649. Our Team Of Volunteers Can: - Help with minor home repairs - Help with basic lawn maintenance - Install assistive devices such as safety grab bars & wheelchair ramps - Assist with accessing services and benefits for those who are eligible - Help with monthly budgeting - And more! Are you an older adult, age 60 or up, who finds it difficult to make ends meet? Are you in need of some help with the cost of groceries or healthcare? You are not alone. Did you know there may be help available for you? Many seniors qualify for—but are not yet enrolled in—benefit programs that could help them pay for prescriptions, medical care, and food. Care Connection links seniors who need help with the resources and volunteers who may provide it. Care Connection is staffed by one part-time COA staff member who coordinates all activities within the department. Everyone else involved in Care Connection is a dedicated COA volunteer. Our volunteers know the value of the work they do and their impact on the lives of those they serve! Some of our dedicated, hard-working Care Connection volunteer groups include: Support Team is the front line of our Care Connection team. They will help you connect with the resources you need, within Council on Aging and in the community. Cutters and Clippers Team aids with basic lawn maintenance to enhance safety, eliminate pests, and avoid unnecessary home owners association fines. Minor Repairs Team assists with minor home repairs, such as changing light bulbs, replacing batteries in smoke detectors, and installing safety grab bars. Accessibility Team designs and builds wheelchair ramps so that those who require them for access can enjoy renewed independence and safety as they enter and exit their homes.
