Bristol Bay Area Health Corporation - Community Health Needs Reports

Bristol Bay Area Health Corporation - Community Health Needs Reports As mandated by the Affordable Care Act (ACA) and as part of the commitment to improving community health, the Bristol Bay Area Health Corporation (BBAHC) collaborates with its partners to complete Community Health Needs Assessments (CHNAs) and their corresponding Implementation Strategies. These reports provide a comprehensive overview of the health needs of the populations served by BBAHC and outline the strategies for addressing these identified needs. Reports are available for download below: 2019-2021 CHNA Report (PDF) 2021-2023 CHNA Implementation Strategy (PDF) Older CHNA Reports are available upon request. A Community Health Needs Assessment (CHNA) is a systematic process involving the community to gather and analyze information about the health of a defined population. It identifies health concerns, disparities, and resources within the community. The goal is to inform public health planning, policy development, and resource allocation to improve the health and well-being of the community. What is a CHNA Implementation Strategy? A CHNA Implementation Strategy outlines the specific actions that organizations and their partners will take to address the health needs identified in the CHNA. It details objectives, activities, timelines, responsible parties, and measures for evaluating progress. These strategies are designed to be practical and achievable, focusing on the most significant health challenges. Community Engagement BBAHC is committed to community engagement throughout the CHNA process. Feedback and input from community members, tribal councils, healthcare providers, and other stakeholders are vital for ensuring that the CHNA accurately reflects the community's needs and that the implementation strategies are relevant and effective. Regular community forums and surveys are utilized to gather this crucial input. Getting a Library Card

Getting a Library Card The Pilot Point Community Library provides free library cards to all Denton County residents, City of Tioga residents, and all Pilot Point ISD students. Services are available to all others for an annual fee. Out of County Residents Individuals who live outside of Denton County may be eligible to receive a library card upon payment of a nonrefundable annual fee. The fees are $15 for an individual card and $20 for a family card. Getting Your Card Present a government-issued photo ID and proof of residency with your current street address. If the address on your ID is current, that may be used as proof of residency. Other proof of residency examples: utility bill, insurance card, personal mail with postmark, etc. Borrowing Limits See the following tables for borrowing limits. All materials check out for 2 weeks.
