About & Contact Resources Reducing Risk and Improving Health and Wellness In 2023, the Tennessee Department of Health began work on a 5-year strategy under a Centers for Disease Control and Prevention program for a "National Cardiovascular Health Program." TDH conducts activities in the following areas to support the program's goals and objectives: Track and Monitor Clinical and Social Services and Support Needs Measures Shown to Improve Health and Wellness, Health Care Quality, and Identify Patients at Highest Risk of Cardiovascular Disease (CVD) with a Focus on Hypertension and High Cholesterol. Implement Team-Based Care to Prevent and Reduce CVD Risk with a Focus on Hypertension and High Cholesterol Prevention, Detection, Control, and Management through the Mitigation of Social Support Barriers to Improve Outcomes. Link Community Resources and Clinical Services that Support Bidirectional Referrals, Self-Management, and Lifestyle Change to Address Social Determinants of Health (SDoH) that Put the Priority Populations at Increased Risk for Cardiovascular Disease with a Focus on Hypertension and High Cholesterol. Cardiovascular Disease Prevention and Management Increase reporting and tracking of clinical data to improve identification, management, and treatment of patients with high blood pressure and cholesterol. Encourage providers to follow evidence-based guidelines and policies for team-based care of patients with high blood pressure and cholesterol. Increase community-clinical links that support systematic referrals, self-management, and lifestyle change for patients with high blood pressure and high blood cholesterol. Encourage patients to continue taking their medicine for high blood pressure and cholesterol. Assist patients with high blood pressure and cholesterol manage their conditions.
