Arkansas readies Medicaid work rules for rural enrollees

LITTLE ROCK, Ark. — The Arkansas Department of Human Services announced a soft implementation of work and community engagement requirements for ARHOME, the state's Medicaid expansion program, starting July 1, 2026. Healthy adults ages 19-64 must log at least 80 hours per month in work, volunteering, education or job training to keep coverage, though no penalties apply until January 1, 2027.[1][2][3] About 217,000 Arkansans, many in rural counties like Phillips in the Delta and Newton in the Ozarks where hospitals have closed, rely on ARHOME for care.[2]
The mandate stems from the One Big Beautiful Bill Act, signed last year by President Donald Trump, requiring states to enforce such rules by 2027.[2] DHS will use existing data to notify enrollees during the July-December outreach if they appear compliant, helping them report activities via a new portal.[1][3] Exemptions cover pregnant women, postpartum mothers, caregivers, disabled veterans and those with medical needs, echoing a 2018 Arkansas policy struck down by federal court after disenrolling thousands.[3][5]
Rural Arkansas faces stark healthcare disparities, with 20% uninsured rates in some Eastern Arkansas counties compared to the state average.[5] ARHOME, launched in 2014 as a market-based expansion, boosted coverage by 3.5 percentage points and annual checkups by 6.1 points versus traditional expansions, narrowing urban-rural gaps.[5] Advocates like Democratic Sen. Clarke Tucker warn reintroducing requirements could reverse gains in places like Helena-West Helena, where clinics serve working poor.[3]
Governor Sarah Huckabee Sanders' administration frames the changes as promoting dignity through work, with DHS emphasizing education over punishment in the soft launch.[3] As implementation nears, rural leaders in Fayetteville and Jonesboro watch closely, fearing administrative hurdles could hit seasonal farmworkers and gig economy participants hardest.
