Ascension St. Vincent's Clay County May Transition to Community Health Partnership

JACKSONVILLE, FL (May 14, 2024) – Ascension St. Vincent’s is exploring transitioning St. Vincent’s Clay County to a Community Health Partnership (CHP) to strengthen access to convenient, personalized care in the community. This model is designed to proactively support health and well-being, improve outcomes, and lower the total cost of care. CHPs offer patients a range of services that go beyond traditional hospital care and emphasize preventative care, chronic disease management and addressing social determinants of health, such as access to healthy food, transportation and housing. They integrate primary care, behavioral health services, and community-based programs to provide comprehensive and coordinated care. “We are committed to ensuring residents of Clay County have access to the right care, in the right place, at the right time,” said Chris Van Deusen, regional president, Ascension St. Vincent’s. “A Community Health Partnership in Clay County would allow us to more proactively support the health and well-being of individuals and families in the community.” Ascension St. Vincent’s has signed a non-binding letter of intent with a potential partner and is engaged in due diligence. A final decision is expected in late summer, with implementation occurring over the next year. During this time, St. Vincent’s Clay County will continue to operate as usual and provide high-quality care to patients. Ascension St. Vincent’s will keep the community informed as the process moves forward.
