West Melbourne Standardizes Backflow Preventer Testing Form

The City of West Melbourne now requires a standardized form for reporting backflow prevention assembly testing and maintenance. Property owners and certified testers can download the 'Backflow Prevention Assembly Testing & Maintenance Report' to streamline the process.

The form gathers detailed premise information, including property name, street address and assembly location. It also requests the backflow preventer's manufacturer, model, serial number and size, along with the meter number and system pressure in PSI.

Testers use the form to record checks on two check valves and a relief valve. For each check valve, testers must note whether it 'Leaked' or 'Closed Tight' during the initial test and the 'Spring Load' if applicable. For the relief valve, the report asks if it 'Opened' at a specific PSI or 'Did Not Open,' and records the 'Buffer Pressure' if it opened.

A section on repairs and replacements allows testers to mark replaced parts, such as rubber parts kits, disc, O-rings, seat, spring, stem/guide, retainer or lock nuts. All repairs or replacements must be completed within 30 days.

After repairs, testers conduct a final test, noting if the check valves 'Closed Tight' and if the relief valve opened at a specified PSI. The testing company and individual tester must certify the report with their certification number and test date.
