Supplemental Information: Submittal package bound into a single pdf document including the following information: Title: A title opinion of a Florida attorney, abstract of title, or other evidence demonstrating property ownership interests in the real property described in the application, which opinion, abstract, or other evidence of ownership is acceptable to the City and covers a period from the day of the date of notice of a change in land development regulation until at least two (2) weeks prior to the date of application for a special permit together with a description of the extent and quality of ownership of the real property during such period. Boundary survey and legal description prepared by a surveyor registered by the State of Florida under a surveyor's seal for the parcel of land upon which the applicant claims to have vested development rights. Affidavit under penalty of perjury executed by the applicant before a notary public attesting to the truth, accuracy and veracity of the application and all attachments thereto. Recital of the Facts: A concise and complete recital of the facts, including dates of expenditures or obligation to expend funds, dollar amounts, the nature of expenditures or obligation to expend funds, and other factors which are claimed to support the claim to a vested right to commence and complete a specific level, type, nature, density, intensity or other form of development. Additional Information relevant to the standards and factors in this section as the Zoning Administrator may specify.

Vegetative Map and Site Plan: Plan with adequate details showing the location of trees and vegetation to be removed which includes the specie or common names and sizes of all trees/vegetation to be removed. If applicable, provide identification of company or individuals who will undertake the tree or vegetation removal. Landscape Plan: A landscape plan is only required if the permit includes property in excess of an acre in size and the property is zoned for other than single-family residential or duplex use unless deemed required by the Building Official. An applicant may choose to submit a landscape plan to clarify the request for the vegetation removal. A landscape plan is required for any new construction or structural expansion or the subdivision of land. Any landscape plan submitted must be drawn at a noted scale of at least 1"=30' and shall include items a through g of the Procedures Manual). The landscape plan must be prepared by and signed and sealed by a landscape architect registered in the State of Florida, unless exempted as outlined in the Procedures Manual. Please note, a stake-out inspection is required prior to any vegetation removal and clearing. All building and paving areas are staked and trees/vegetation to remain must be tagged and barricaded. Topography Plan (includes items 1 through 3 of the Procedures Manual). Building Elevations for all sides including structural heights, proposed building materials list and color renderings. Existing Building Uses (if any) and intended use of each building or portion thereof. Level of Service Data and Analysis (includes items 1 through 4 of the Procedures Manual). Soils Map depicting detailed soils report based on the findings of a professional soil expert supporting proposed construction activity. Signs and Lighting Specifications showing the existing and proposed locations, elevations and typical sections and the height, overall square footage for each sign proposed. Wildlife Inventory (if the site is previously undeveloped), an inventory of wildlife species on site including species categorized as endangered, threatened or of special concern status as well as a description of the techniques and practices proposed to protect endangered, threatened of special of special concern status.
