The schedule of charges as described in above sections shall be adjusted on meter readings starting on October 1st, beginning 2015 and each subsequent year by the amount of increase in the South Urban Unadjusted Consumer Price Index (CPIU) as published by the Bureau of Labor Statistics (http://data.bls.gov/pdq/SurveyOutputServlet?series_id=CUUR0300SA0,CUUS0300SA0 ) by amount reported, but NOT to exceed three (3%) percent (whichever is less) for the preceding calendar year. IF the CPIU declines during the previous calendar year the rate adjustment will be zero (0%) percent.* This adjustment only effects the consumption water rates for retail inside and outside customers, commercial, large commercial/industrial, and wholesale customers. * Example: If CPIU for 2014 equals 2.0% for the calendar year (January to December), the rates will increase 2.0% in October of 2015. If CPIU equals 3.25% for the 2014 calendar year the rates will increase a maximum of 3%. If the CPIU for the calendar year of 2014 drops by -1%, the rates will remain unchanged. A copy of the revised consumption for rates shall be posted on the Clarksville Connected website (www.clarksvilleconnected.net). The Clarksville Connected Commission shall be authorized to regulate the percentage (%) adjustment on the annual basis at a percentage (%) below the published CPIU increase if the retained earnings of the utility remains at a reasonable and sustainable amount based upon its annual budgetary projections. 400 W Main Street P.O. Georgia West, Winston-Salem Theatre Volunteer and Community Leader, 93

Georgia Nicolas West, age 93, died on May 10, 2026 after several years of declining health. She was born in Augusta, GA on August 21, 1932 to the late Alexis Ravenez and Rosalyn Poole Nicolas. Her family moved to Winston-Salem in 1936, where she remained a resident until her death. She graduated from R. J. Reynolds High School in 1950 then matriculated to Woman’s College (now UNCG) where she graduated with a BA degree in Biology in 1954. After graduating college, she began work at Western Electric as a technical assistant in the lab section. While working there, she was introduced to an electrical engineer named Warren F. West Jr. The introduction was the beginning of a lifelong relationship full of love and devotion. He proposed on March 26, 1955 and they were married on August 20, 1955. She resigned from her job about a year later to focus on building a family. They had four children, Ellen Lynn, Warren III, William Glenn, and John Brannon.Georgia was a dutiful homemaker and mother. While raising four children requires considerable effort, she still managed to pursue interests outside the home. One significant interest was her lifelong love of theatre. It was multi-faceted. She enjoyed attending shows, acting, and volunteering with makeup design. Georgia was involved with The Little Theatre of Winston-Salem for over 5 decades - Acting in multiple shows, and providing makeup support for many others. She assisted with other theatre aspects, from house manager, box office, and archive preservation. She was a member of the Board of Directors for many years, serving as Board President from 1982-1984.While Georgia was proud of her involvement with LTWS, she felt there was support in the community for even more theater. In 1983, she was part of a group who founded the Winston-Salem Theatre Alliance. Her level of involvement with WSTA was similar to that with LTWS. Over the years, WS Theatre Alliance has grown into one of the prominent theater groups in town. Georgia enjoyed participating with multiple theater groups, and seeing their growth in the city. In the late 80s, Georgia and Kay Johnson started Johnson-West Associates, where she was a talent agent and assisted many local actors to find paying work. She was also involved with production of Carolina Street Scene and the Crosby Clambake.Georgia was a dedicated member of First Presbyterian Church, having served as a Sunday School leader, and coordinator of many fellowship entertainment shows. She volunteered teaching Creative Dramatics at Brunson Elementary. She enjoyed traveling, particularly after Warren’s retirement. He and Georgia regularly traveled to Europe and many parts of the US. Many of the trips included the children and grandchildren, making lots of memories.Georgia was preceded in death by her parents, her husband Warren F West Jr, her brother Alexis Ravenez Nicolas Jr, and her sister Rosalyn Nicolas MacDonald. She is survived by her 4 children, Ellen West Law, Warren F West III (Marsie), W Glenn West (Becki), and John B West (Ann) of Charlotte; 7 grandchildren, Fletcher West, Nicolas West (Hannah), Grace West (Nicolas Stougaard), Matthew West, Connor Law (Mandy), Paige W Krantz (Robert), and Brannon West (Valentina); 3 great grandchildren, Margaret, Bella, and Eleanor; and numerous nieces, nephews and cousins. The family would like to thank the staff at Homestead Hills for all their love and care during Georgia’s six years there. A celebration of life will be held at First Presbyterian Church at 300 N Cherry St in Winston-Salem on Saturday, June 6th at 2pm. A private committal service for the immediate family will be held prior to the celebration. In lieu of flowers, donations may be made in Georgia’s honor to the Little Theatre of Winston-Salem, Winston-Salem Theatre Alliance, First Presbyterian Church, or the charity of your choice.

Dr. John Faris, Winston-Salem Radiologist, 85

After a decade-long battle with dementia, John Charles Faris passed away on May 26, 2026, at KBR Hospice Home in Winston-Salem. He was born on March 24, 1941, in Centralia, Illinois, to James Harold Faris and Margaret Rohde Faris. He graduated in 1959 as a member of the final class of Central High School in Charlotte, North Carolina. He then attended Duke University, majoring in pre-med, and graduated in 1963 before entering Bowman Gray School of Medicine, now the Wake Forest University School of Medicine. After earning his medical degree in 1967, he completed a one-year general surgery residency at Denver Presbyterian Hospital in Denver, Colorado. He then returned to Winston-Salem to begin a radiology residency. Like many physicians of that era, his training was interrupted during the buildup to the Vietnam War because of his Air Force commitment through the Berry Plan. With one year of radiology training completed, he attained the rank of Major and served as a medical officer and radiologist at Castle Air Force Base in Merced, California, and at Royal Thai Air Force Base in Udorn, Thailand. The Royal Thai Air Force Base served as a front-line base for the United States Air Force and as the Asian headquarters for Air America. After completing his Air Force service, John returned to Winston-Salem, driving his brand-new 1970 cherry-red T-top Chevrolet Corvette. While finishing his residency in 1973, he met his future wife, Ann, during a spirited disagreement over a patient’s care. They crossed paths again a month later when John suffered a ruptured appendix. Family and friends will have no doubt about who won the original disagreement, even though John was the one who let his appendix rupture. After completing his residency, John began a private practice in diagnostic radiology that spanned more than 40 years. He was instrumental in bringing state-of-the-art mammography to Forsyth County and the surrounding area, leading to earlier detection of breast cancer and saving countless lives. He also established the community’s first mobile mammography diagnostic service and introduced its first bone densitometry scanner to help diagnose osteoporosis and monitor treatment. His legacy of compassionate care for patients undergoing breast cancer diagnosis continues to guide his former colleagues at the Breast Clinic and Forsyth Radiological Associates.John loved watching Duke basketball, snow skiing, and especially golfing with his “Gator Cup” buddies, with whom he was also known for his salad-making skills. He is survived by his wife of 50 years, Ann; his son, Justin Charles Faris (Kimberly); his daughter, Jessica Elizabeth Jones (Rob); his grandchildren, Taylor, Isabella, Connor, and Caleb; his great-grandchild, Blake; and his sister, Judy Ogburn of Georgetown, South Carolina. The family extends their heartfelt gratitude to Trellis Palliative Care, KBR Hospice Home, and to Tia Mayfield and Iris Ortega, who lovingly cared for John in his final months.A remembrance service for family and friends will be held in the Chapel at Centenary United Methodist Church at 3:00 p.m. on Friday, May 29. Following the service, the family will move to the columbarium for a brief inurnment service. The family will greet guests after the committal in the Dining Room, located just outside the Chapel entrance. Please use the 4&1/2 Street entrance for easy access to the Chapel and Dining Room.In lieu of flowers, memorial gifts may be made to Senior Services, Inc. in support of the Faris Family Fund for Caregiver Education and Support. Gifts may be mailed to 2895 Shorefair Drive, Winston-Salem, NC 27105, or made online at seniorservicesinc.org, with the fund name noted in the comments field.
