The form collects the following information: Name, Email Address, Phone Number, and Library Card Number. Patrons can indicate whether items can be requested from another Monarch library. The form asks patrons to list some of their favorite books, favorite authors, and any authors they are interested in reading. Preferences collected include: book length (shorter books under 400 pages, longer books over 400 pages, or no preference), format preference (fiction, nonfiction, or both), and format type (Standard Print, Large Print, or Audiobook). Patrons can select preferred genres and topics including: Mystery, Fantasy, Romance, Science Fiction, Thriller, Western, Historical Fiction, Graphic Novels, History, Science, Biography, and Self Help. Additional fields allow patrons to note any authors, books, or genres to avoid and provide any additional information for the personal shopper.
