Births - October 1, 1907, to present. Marriage - October 1, 1907, to present.  Deaths - September 1, 2013, to present. Prior to September 1, 2013, you will need to contact the County Register of Deeds where the death occurred.  Divorce - January 1, 2016, to present. Prior to January 1, 2016, contact the County Clerk of Courts where the divorce occurred. For Walworth County, please call 262-741-7012 Applicants must have a direct and tangible interest in order to purchase a certified copy of any vital record and must provide current identification when submitting the application. Submit Applications in Person Applications submitted in person at our office generally take 15 minutes to complete. We ask that you arrive by 4:15 p.m. to allow time to fill out the application. You must present current, valid identification at the time of application.  Identification Requirements Please bring one of the following:  State-issued driver's license or ID card                                                U.S. government-issued photo ID U.S. or foreign passport Tribal or military ID card or two of the following: Bank/earnings statement Current, dated, signed lease Health insurance card Utility bill or traffic ticket Vehicle registration/title Matricula Consular ID card Submit Applications by Mail Complete the appropriate form(s) below, include a copy of your identification, a self-addressed stamped business-sized envelope, and the applicable fee.
