Many men in North Miami Beach may notice changes in urinary habits as they age, such as more frequent bathroom trips, a weaker stream, or difficulty fully emptying the bladder. These symptoms often prompt concerns about an enlarged prostate, a common condition known as benign prostatic hyperplasia (BPH), especially noticeable during leisurely walks along the nearby Oleta River State Park.

While prostate growth is a natural part of aging, understanding when its size becomes a medical concern is crucial. The prostate, typically the size of a walnut in younger adults, gradually enlarges over time. A prostate measuring 30 to 40 grams is generally considered mildly enlarged. However, when it reaches 60 grams or more, it can begin to interfere with urine flow, leading to lower urinary tract symptoms (LUTS). A prostate of 80 to 100 grams or larger is often deemed significantly enlarged, especially if it causes obstruction or discomfort.

Beyond size, doctors become more concerned when an enlarged prostate leads to symptoms such as frequent urges to urinate (especially at night), a weak or interrupted urine stream, difficulty starting urination, post-urination dribbling, a sensation of incomplete bladder emptying, or urinary incontinence.

An excessively large prostate can lead to serious complications if left untreated. These include urinary retention (inability to urinate without a catheter), bladder stones, blood in urine (hematuria), recurring urinary tract infections, and even kidney damage or failure due to backed-up urine. The prostate can continue to grow, significantly impacting quality of life.

Prostate size is typically measured using imaging techniques like transrectal ultrasound or MRI. Most BPH-related growth occurs in the transitional zone of the prostate, which is why symptoms can vary even with similar overall prostate sizes.

When symptoms worsen or imaging reveals significant tissue compression of the urethra, treatment may be necessary. Options range from medication to relax the prostate or reduce hormone-driven growth, to surgical treatments like transurethral resection of the prostate (TURP). A newer, minimally invasive option available in Miami is prostate artery embolization (PAE).

PAE is an outpatient procedure performed by an interventional radiologist. It involves reducing blood flow to targeted areas of the prostate, causing the gland to shrink over time. This procedure requires only a tiny puncture site, typically in the groin or wrist, and uses local anesthesia. It aims to preserve sexual function and has a low risk of retrograde ejaculation, offering a quicker recovery compared to traditional surgery.

Men with moderate to severe BPH symptoms, those who are not good candidates for surgery, or those seeking a procedure with faster recovery and fewer risks may be candidates for PAE. Regular prostate screenings, particularly after age 50, are important for monitoring growth and detecting any rapid increases in size or signs of potential prostate cancer, which may require further testing like PSA blood tests, digital rectal exams (DRE), or biopsies—especially beneficial in North Miami Beach's active senior communities near the beaches.

Residents experiencing persistent urinary symptoms should consult their healthcare provider to determine if their prostate size warrants action and to explore appropriate treatment options.
