Cornerstone is proud to offer services for all youth living in Lake, Sumter, Orange, Osceola, Polk, Highlands, and Hardee counties. Cornerstone’s Kids Pediatric Care It is a difficult emotional decision to make, enrolling your child in hospice care. Cornerstone is here to guide you and your family through the process. Our skilled team are available to support you in navigating the challenges that surface throughout care. Nursing care, social worker, spiritual care and hospice aides are each available to support the needs and care plan that you create. This is your families plan and together we will work to ensure it is followed. To find out the resources available and if hospice care is right for you, call Cornerstone to discuss 866-742-6655. If your child is receiving medical coverage through Medicaid or the Children’s Insurance Program read the following information on additional resources available. Concurrent Care Concurrent Care is a program available to children under the age of 21 with a life limiting illness. The program provides access to children on Medicaid or the Children’s Insurance Program to continue receiving aggressive treatment, while simultaneously accessing the benefits of Cornerstone’s hospice services.
