IMMUNIZATION REQUIREMENTS FOR SCHOOL ATTENDANCE
IMMUNIZATION REQUIREMENTS FOR SCHOOL ATTENDANCE All students attending Florida schools are required to be vaccinated and provide documentation of those vaccinations on the Florida Certificate of Immunization (DH Form 680), to the school for entry, attendance, or transfer. A valid exemption from immunization must be documented on the appropriate Florida Department of Health form. The Florida Department of Health in St. Lucie County provides immunizations. Required Vaccines: • Diphtheria-Tetanus-Pertussis vaccine (DTaP, DT, Tdap, Td) – 4-5 doses • Inactivated Poliovirus vaccine (IPV) – 3-4 doses • Measles-Mumps-Rubella vaccine (MMR) – 2 doses • Varicella vaccine (chickenpox) – 2 doses • Hepatitis B vaccine (Hep B) – 3 doses • Meningococcal Conjugate Vaccine (MCV4) – 1 dose, required for 7th-12th grades Where to Obtain Immunizations: • Your child’s healthcare provider • Florida Department of Health in St. Lucie County (Services may vary.
