Senior Case Manager - Thomas Merton Family Center Senior Case Manager Full-time - Benefits eligible Thomas Merton Family Center | Bridgeport, CT Deliver structured and compassionate case management services, supervise junior staff, coordinate services, and serve as a key link between clients, community resources, and internal services at TMFC. Accountabilities Conduct client intake assessments and develop goal-oriented care plans. Support clients with referrals, paperwork, and service access. Provide advanced client support including home visits and grocery delivery as approved. Maintain accurate case documentation and reporting. Supervise and mentor Junior Case Managers, delegating tasks appropriately. Participate in team meetings, training, and collaborative case reviews. Education & Experience Bachelor’s degree in social work, Human Services, or related field; equivalent experience considered. 3–5 years case management experience, preferably community-based or nonprofit. Experience with at-risk populations and data entry. Spanish or Haitian Creole preferred; Microsoft 365 proficiency a plus.
