Lee County Special District Candidate Packet and Forms The next Election Cycle for Special District Offices is 2026. Scroll down for online candidate packet contents available for download. Candidates may also obtain Forms and Publications by visiting the Florida Division of Elections website. 2026 Special District offices include: Captiva Erosion Prevention District, Community Development Districts, Fort Myers Beach Public Library District, Fort Myers Beach Mosquito Control District, Independent Fire Control and Prevention Districts, Lee County Mosquito Control District, and the Sanibel Public Library District. Candidate Petition Deadline Noon, May 11, 2026 Candidate Qualifying Dates Noon, June 8 through Noon, June 12, 2026 Candidates may pre-qualify beginning on May 25, 2026. (Our office will be closed May 25 for the Memorial Day holiday.) Candidate Qualifying Forms Lee County Special District Candidate Fact Sheet (PDF) Candidate File Cover Sheet (PDF) (Initial Filing - Required) Affidavit of Intent - Special District Candidate (PDF) (Initial Filing - Required if not submitting DSDE 9) Text Messaging Service Agreement (PDF) (Optional, but Recommended) DSDE 9 Appointment of Campaign Treasurer and Designation of Campaign Depository (PDF) (Initial Filing - Required only if the candidate will actively campaign) Any special district candidate who intends to accept contributions or make expenditures in connection with his/her campaign must appoint a campaign treasurer, designate a campaign depository, and file periodic campaign treasurer's reports with the Lee County Supervisor of Elections. Contributions and expenditures include any made by the candidate in connection with his/her campaign. DSDE 84 Statement of Candidate (PDF) (Initial Filing - Required) Campaign Financial Reporting System Affidavit (PDF) (Initial Filing - Required) The Lee County Supervisor of Elections provides its own (CFR) Campaign Financial Reporting System for the filing of required campaign finance reports. The Lee County (CFR) Campaign Financial Reporting System facilitates the electronic filing of required campaign finance reports, replacing the need for original and signed reports to be filed by hand delivery or standard mail. Upon filing Form DS-DE 9, a candidate and his/her campaign treasurer and deputy treasurer (if one is appointed) will be provided a User Identification Number, password, and PIN access to the CFR System for the filing of required campaign treasurer's reports. DSDE 104 Candidate Petition Form (PDF) (Optional) A candidate for Special District may obtain the valid signatures of 25 registered electors within the district. Submit signatures to the Lee County Supervisor of Elections for validation. Upon certification by the Supervisor of Elections that the candidate has obtained the required number of valid signatures, the signatures shall serve as the filing fee. Form 1 Statement of Financial Interests, Candidate Oath, Canvassing Board Meeting Schedule and Filing Fee (If Any) Form 1 Statement of Financial Interest for 2025 (Required - submit at the time of qualifying) Form 1 must be filed electronically with the Florida Commission on Ethics via the Electronic Financial Disclosure Management System (EFDMS).A copy of the electronically filed Form 1 must be provided with your final qualifying documents. DSDE 302NP-E Corr Candidate Oath (PDF) - Non-Partisan Office (Required - submit at the time of qualifying) A special district candidate must file the correct Candidate Oath for the office they are seeking. This form may only be submitted during the qualifying period. Canvassing Board Meeting and Logic and Accuracy Testing Schedules (Required - submit at the time of qualifying) Filing Fee (PDF) (Required - submit at the time of qualifying) $25 filing fee. A special district candidate may pay the filing fee with a personal check, cash, money order, or campaign check (if a campaign account has been established at a financial institution). If paying by personal check, money order, or campaign check, make the check payable to the Lee County Elections Office for the exact amount. A candidate who obtains the required number of valid candidate petition signatures for the office sought will not be required to pay a filing fee.
