We’ll ensure your student receives recommended vaccines based on their age and immunization history. Available vaccines: Flu, Tdap, HPV and meningococcal. A parent or guardian must complete the immunization and questionnaire consent form before a student can receive vaccines. Students can bring the form to their visit or send it electronically to [email protected].
