The sponsorship application form requests the following information: - Business Name - Your Name (First and Last) - Address (Street Address, City, State, ZIP Code) - Contact Information (Email and Phone Number) - Preferred Method of Contact (Email or Phone) - Event(s) Interested in Sponsoring: Family Events, Youth Events, Cultural Events, Senior Events, Patriotic Events, or Adult Events (21+) - Consent certification that applicant is over 18 years old and understands the information provided is a public record pursuant to Chapter 119 of Florida State Statute
