Doris Ann Worman, 79, of Cincinnati, passed away peacefully on April 10, 2024. Born in Cincinnati on July 15, 1944, Doris was the cherished daughter of the late Arthur and Eleanor Schmidt. She was a graduate of Western Hills High School and dedicated many years to her career as a librarian at the Cincinnati Public Library, where she fostered a love of reading in countless children. Doris was a devoted mother to her son, David Worman (Sarah), and a loving grandmother to Emily and Michael. She was preceded in death by her husband, Robert Worman, and her brother, James Schmidt. Doris was an active member of St. Mary's Catholic Church and enjoyed gardening, reading, and spending time with her family. A Mass of Christian Burial will be held at St. Mary's Catholic Church, 1030 Herron Ave, Cincinnati, OH 45211, on Monday, April 15, 2024, at 10:00 AM. Interment will follow at Spring Grove Cemetery. In lieu of flowers, the family requests donations be made to the Cincinnati Public Library Foundation in Doris's memory.

We are here for you. Vital Statistics Worksheet To file the death certificate, we need the deceased’s authorized agent to fill out and complete the Vital Statistics Worksheet. Please return the form to us within five days to comply with Ohio law. Download Form Appointment of Representative (Before Need) In Ohio, the deceased’s legal next-of-kin has the right to make the funeral arrangements. If you would like someone else to arrange your funeral, you need to appoint that person by filling out an Appointment of Representative for Disposition of Bodily Remains, Funeral Arrangements, and Burial or Cremation Goods and Services form.
