Holy Trinity - Athletic Clearance Guide

Holy Trinity - Athletic Clearance Guide Athletic Clearance is required for students participating in Upper School Athletics. Required forms FHSAA EL2 (Preparticipation Physical Evaluation): All new physicals must be completed on the FHSAA EL2 form, and only page 4 should be uploaded to Holy Trinity’s Online Athletic Clearance. The physical exam must be documented on the EL2 form for athletics; no other forms will be accepted. Electrocardiogram (ECG): For the 2026-2027 school year, an electrocardiogram (ECG) is required to participate in athletics at Holy Trinity. Each student-athlete must have a completed EL1 form and upload it to their athletic clearance account. An ECG completed on or after July 1, 2024, will satisfy this requirement. If additional evaluation by a specialist is required, a completed Medical Eligibility Supplemental Form (EL1/2S) must be submitted before athletic participation is permitted. NFHS courses: Certificates of Completion for Concussion, Heat Illness Prevention, Sudden Cardiac Arrest, Hazing Prevention, and Sportsmanship must be uploaded to Holy Trinity’s Online Athletic Clearance. GA4: This form is only required for student-athletes (entering grades 10-12) transferring from another high school. HT’s Online Athletic Clearance Visit www.AthleticClearance.com and select the button for Florida. First time users: Create an Account. Parents/guardians should register with a valid email address and password (student email addresses are not appropriate). Returning users: Sign in using your login information. Click on the “Start Clearance Here” button on the top right corner and follow the prompts to begin your athletic clearance. Be sure to select ALL of the sports that your student plans to try out for. Compete all required fields: Student Information (including student ID located on student contact card in onCampus) Parent/Guardian Information Medical History Signature Forms (names must be exact matches, case-sensitive) Upload required forms and proof of insurance You will receive a confirmation message once you have completed the online registration process. PLEASE NOTE: the student is NOT cleared yet. This data will be electronically filed with Holy Trinity’s athletic department for review. An email notification will be sent once the student has been cleared or denied for participation. Please be mindful of processing time as each clearance must be reviewed individually. 2026-2027 Baseline Concussion Test – take the baseline test at home What is Impact? The ImPACT test is a computer – based neurocognitive test battery that provides healthcare professionals with objective measure of neurocognitive functioning as an assessment aid and in the management of concussion in individuals ages 12-59. The ImPACT Baseline test can be completed at home. Please make sure of the following before continuing your test: Test environment must be quiet and distraction-free, no phones or other devices. Sit comfortably at a table or desk with a flat, hard surface. All other programs and browser tabs should be closed. An external mouse or a trackpad must be used throughout the test. Touch screens are not acceptable. You must complete the test in a single attempt in less than 45 minutes. Get Started: Launch the ImPACT Testing Website: Enter code 7ZXR Select Validate Select Launch Baseline Test If your student athlete is suspected of having a concussion during the school year, you can choose to share the ImPACT Baseline results with a concussion care provider. During the visit, the student will take a post-injury ImPACT test administered by the provider. The results will be compared with the Baseline test to help determine the severity of the concussion and guide the recovery process. FAQs What is my username? Your username is the email address that you registered with. How do I register for multiple sports? If you know your athlete plans to play multiple sports when registering, it is best to add all sports on the first step where you also select the school year and school. If you are registering for additional sports after completing your initial clearance for the year, you will have to complete the process again. Does my child have to get a heart screening? For the 2026-2027 school year, an electrocardiogram (ECG) is required to participate in athletics at Holy Trinity. Each student-athlete must have a completed EL1 form and upload it to their athletic clearance account. An ECG completed on or after July 1, 2024, will satisfy this requirement. If additional evaluation by a specialist is required, a completed Medical Eligibility Supplemental Form (EL1/2S) must be submitted before athletic participation is permitted. What if my athlete is transferring from another high school? You must upload the required GA4 form when completing your online clearance if your athlete participated in athletics at another high school. Where do I go to complete the required NFHS courses? These courses, presented by the National Federation of State High School Associations (NFHS) are free but require an email login. Each course provides a completion certificate which must be uploaded when completing an athletic clearance. Courses include: Concussion, Heat Illness Prevention, Sudden Cardiac Arrest, Hazing Prevention, and Sportsmanship Why hasn’t my athlete been cleared? Holy Trinity will review the information you have submitted before clearing your athlete for participation. Once your clearance has been reviewed, your clearance status will be updated. You will receive an email notification when your athlete has been cleared or denied for participation. My athlete was “denied” clearance, now what? You should have received an email with the reason for denial. Please update your clearance accordingly, then contact Holy Trinity’s Athletics Department to review your information again. Schedule a Redelivery If we missed you when we tried to deliver your mail, you can schedule a Redelivery online using a tracking number or the barcode number shown on the back of your PS Form 3849, We ReDeliver for You! Redeliveries can be scheduled online 24 hours a day, 7 days a week. For same-day Redelivery, make sure your request is submitted by 2 AM CST Monday-Saturday or your Redelivery will be scheduled for the next day. Check to determine if Redelivery is available for your address. Step 1: Check if Redelivery is available for your address. Please provide your contact information below. The address must match the original delivery address. Required fields include First Name, Last Name, Street Address, City, State, ZIP Code, Phone, and Email. Step 2: Select packages for Redelivery. Enter a tracking or barcode number shown on the back of your PS Form 3849, We ReDeliver for You! All packages in a single Redelivery request must be associated to the same PS Form 3849. Tracking numbers are for an individual package. Barcode numbers can have multiple tracking numbers associated with it. Both tracking and barcode numbers are between 13 and 34 characters.
